* gpgsm.h (OUT_OF_CORE): Removed and changed all callers to
out_of_core.
(CTRL): Removed and changed everywhere to ctrl_t.
(CERTLIST): Ditto.
Replaced all Assuan error codes by libgpg-error codes. Removed
all map_to_assuan_status and map_assuan_err.
* gpgsm.c (main): Call assuan_set_assuan_err_source to have Assuan
switch to gpg-error codes.
* server.c (set_error): Adjusted.
