scd,pcsc: Support multiple card readers.
* scd/apdu.c (close_pcsc_reader, apdu_init): Clear pcsc.rdrname. (pcsc_init): Load of PC/SC module moved from ... (open_pcsc_reader): ... here. (apdu_dev_list_start): Add support for PC/SC. (apdu_dev_list_finish): Likewise. (apdu_open_reader): Likewise. Signed-off-by: NIIBE Yutaka <gniibe@fsij.org>
