SCD: Support fixed length PIN input for keypad (PC/SC).

* scd/apdu.c (pcsc_keypad_verify): SUpport fixed length PIN input for
keypad.
(pcsc_keypad_modify): Likewise.
* scd/ccid-driver.c (ccid_transceive_secure): Clean up.
